Below you will find all the forms you will need to enroll your child. These forms are designed to be typed directly on and then printed out. You can drop them off at our center or mail them. Please feel free to give us a call if you have any questions.

School supplies
The following list of items should be brought with your child on their first day at Presentation ECC. This list applies to all ages unless otherwise indicated. Please make sure all items are clearly labeled and given directly to your child’s teacher. (We ask that you do not send toys or candy to school with your child.)
- Change of clothes (season appropriate, including socks)
- Water bottle (label and keep at school)
- Sunscreen
- Nap items
- Diapers (toddlers only; replenished as needed)
- Baby wipes (toddlers only; replenish as needed)
- Sippy cup (toddlers only)
- Family picture for classroom
- Medication if needed (must be left at front office)
• travel pillow
• crib sheet (must have some sort of sheet to cover cot)
• blanket
• favorite item (stuffed animal, book etc.)
• bag to store items (should be able to hang on hook outside of classroom and be able to travel home once a week)
